乳がんは全身病と考えられています。
すなわち全身を循環しているかもしれないがん細胞を如何にコントロールするかが大切です。
今日はそこらへんのお話です。

-Circulating tumour cells（循環腫瘍細胞）
Circulating tumour cells have been increasingly studied as poor prognosis markers (though they are not yet ready for routine use).
循環腫瘍細胞（CTC）は予後不良因子として多く研究されている。（まだルーチンで使用できる段階ではない）
New technology allows the evaluation of phenotypic markers in individual circulating tumour cells and has demonstrated that these may differ from gross characteristics of the parent tumour .
新しい技術によって、個々の循環腫瘍細胞の表現型マーカーを選別できるようになり、患者の腫瘍の全体的な特徴と相異なる場合があることが証明されている。
Thus, for example ,HER2 overexpression in circulating tumour cells might justify targeted therapy even in the absence of conventional HER2 positivity of the primary tumour.
それ故、例えば主病巣がHER2陰性であっても、循環腫瘍細胞においてHER2の過剰発現が見られれば標的治療の対象となる可能性がある。
This strategy is undergoing clinical investigation.
この戦略は現在、研究段階である。
Current studies are examining the possibility that some circulating tumour cells may represent breast cancer stem cells.
最近の研究ではある循環腫瘍細胞は乳がん幹細胞に相当するかもしれないという可能性を調べている。

今日は乳がんに関する討論会が開催されました。
1チーム　4人ずつ、2チームに分けて、ディベートが行われました。
私も大きな声で、みんなに自分の治療方針について、おしゃべりをしてきました。
どちらかというと、大きな声でみんなの前でお話するのは嫌いではありません。
ですが、神奈川県内の乳腺の先生がたくさんおいでになっている、そんな緊張感はたまりませんでした。
でも、患者さんのみなさんを代表して、この患者様にはこの方法が最も大切、ということを先生方に伝えてきました。
